Application Instructions

The application process includes two steps:
1. Application submitted to International Services for an OPT I-20.
2. Application returned to applicant. Applicant will mail application materias to USCIS.

Students can have USCIS notify them of receipt of their application via email or text message by submitting the Form G-1145 along with the application. This form should be placed on top of the application sent to USCIS.

1. APPLICATION TO OIS
Based on what OPT you are applying for submit the below listed documents to OIS:

PRE- COMPLETION OPT INSTRUCTIONS: (Please do not staple your application materials)

  • Form G-1145
  • Completed General Request Form (complete top section and part C)
  • A copy of your unofficial transcript (from your School's Registrar's office) - for verification of a full-time course load / maintenance of F-1 status;
  • A completed Form I-765. Read the I-765 Instructions carefully. The code for question 16 is: (c) (3) (A)
  • 2 Photographs - passport style. On the back of the photos print your name and I-94 number in pencil.
  • Copies of all previous I-20s (page 1 & 3 for old form I-20, page 1 & 2 for new form I-20)
  • Copies of your passport biographical page (include passport expiration date page if separate), most recent visa and I-94 (front and back).
  • A letter of recommendation for OPT from your school's Registrar addressed to Ms. Stephanie Otte. The letter should also state your expected program completion date.
  • A check for $410 made to US Department of Homeland Security.
  • A copy of any previously issued EAD (if applicable).

POST COMPLETION OPT INSTRUCTIONS: (Please do not staple your application materials)

  • Form G-1145
  • Completed General Request Form (complete top section and part C)
  • A copy of your unofficial transcript (from your School's Registrar's office) - for verification of a full-time course load / maintenance of F-1 status;
  • A completed Form I-765. Read the I-765 Instructions carefully. The code for question 16 is: (c) (3) (B)
  • 2 Photographs - passport style. On the back of the photos print your name and I-94 number in pencil.
  • Copies of all previous I-20s (page 1 & 3 for old form I-20, page 1 &2 for new form I-20)
  • Copies of your passport biographical page (include passport expiration date page if separate), most recent visa and I-94 (front and back).
  • A letter of recommendation for OPT from your school's Registrar addressed to Ms. Stephanie Otte. The letter should also state your expected program completion date. (The completion date is the day on which you complete all degree requirements, NOT the day that you attend commencement and NOT the day your degree is awarded). If the letter from your Registrar does not state this date, additional letters may be necessary.
  • A check for $410 made to US Department of Homeland Security.
  • A copy of any previously issued EAD (if applicable)

Special Note

  • POST COMPLETION ONLY: Completion Date vs. Graduation Date: There are many different dates marking the end of a degree program. However, for immigration purposes and Post-completion OPT the date to mark the end of the academic program is you COMPLETION DATE (see definitions below)
    • Completion Date: Date complete all degree requirements (i.e. last day of finals, date submit final draft of Ph.D. dissertation to department). USE THIS DATE FOR POST-COMPLETION OPT
    • Graduation Date: Arbitrary date RBHS assigns to your degree completion for administrative purposes. Not used for OPT purposes.
    • Ceremony Date: Date at end of Spring term when Commencement exercises are held. Not used for OPT purposes.

STEM OPT INSTRUCTIONS (Please do not staple your application materials)

  • Request for 24 Month Extension Recommended I-20
  • Form G-1145
  • A copy of your diploma
  • A completed Form I-765. Read the I-765 Instructions carefully. The code for question 16 is: (c) (3) (C). For question 17, contact employer for E-Verify registration number.
  • 2 Photographs - passport style. On the back of the photos print your name and I-94 number in pencil.
  • Copies of all previous I-20s (page 1 & 3 for old form I-20, page 1 &2 for new form I-20)
  • Copies of your passport biographical page (include passport expiration date page if separate), most recent visa and I-94 (front and back).
  • Copy of current unexpired EAD card
  • A check for $410 made to US Department of Homeland Security.
  • Letter of employment verification from your employer.
  • Statement from student on jobs relevance to program.
  • I-983 Training Plan

2. APPLICATION TO USCIS
Once you prepare all of your documents for your OPT applicaiotion, call OIS to make an appointment with an international advisor to complete the application process and issue a new form I-20. After the appointment you can mail out the application to USCIS. OPT application must be submitted to the USCIS Processing Center within 30 days of the date the school enters the recommendation for OPT into the SEVIS record.

USCIS Processing Time:
USCIS' normal processing time is 90 days from the date your application is receipted.
