WHAT DOCUMENTS WILL I RECEIVE AFTER APPLYING FOR AID?

If you have a current email on file, and your FAFSA was processed successfully, you will receive an email notification from the Central Processor with instructions on how to access your electronic SAR (Student Aid Report). If you do not have an email address, or you submitted a paper FAFSA, you will receive a paper copy of the SAR.

If you would like to print a copy of your SAR, you can visit www.fafsa.ed.gov and request one by selecting Print SAR from the left navigation bar. Please note: you will need your PIN code to access this request form.

You may also receive a request for supplemental documents from the Financial Aid Office. Some of the documents requested are for reference; others are required to determine your level of eligibility or to disburse financial aid.

FORMS AND CORRESPONDENCE
Missing Information Letters
Once the completed FAFSA has been received by our institution, a Missing Information Letter will be generated if we have not received all required documentation.

Incoming Students:
A Missing Information Letter will be mailed to the student's most current mailing address on file. (The Financial Aid Application asks where you want your Financial Aid mail to be sent.) It will detail the documents/questions necessary to complete processing your file. UMDNJ Financial Aid Forms are accessible online to print by clicking here for Useful Forms.

A copy of your signed federal income tax return, with all schedules and pages, or Benefit Letters from agencies must be sent into the Financial Aid Office.

All requested documents should be mailed to the Office of Student Financial Aid requesting them.

After the applicant has been registered and has been provided a UMDNJ email address, he/she will be advised electronically to visit the myPortal and view his/her missing information.
 
Continuing Students:
Will be advised electronically to check the myPortal and view their Missing Information Letter.

UMDNJ Financial Aid Forms are accessible online to print and complete (click here for Useful Forms).

All requested documents should be mailed to the Office of Student Financial Aid requesting them.

Verification Worksheet
If you are selected for verification by the U.S. Department of Education, you will be required to complete and submit a VERIFICATION WORKSHEET to the Student Financial Aid Office.

Dependent Childcare Expense
For students who have expenses for dependent children or family members, additional documentation will be required.

Internal Revenue Release Form
The Student Financial Aid Office may determine that a copy of your federal income tax be sent directly from the IRS. If this is the case, an IRS RELEASE FORM will be mailed to you for signature.

Budget Re-Evaluation Form
This form is to be completed by students who expended in part or whole, their prior year earnings on the tax return. These expenses should be itemized on this form, and documentation for each must be attached.

Student's Non-Tax Filing Statement
For students who did not file federal income tax and were not required to.

Parents' Non-Tax Filing Statement
For parents of students who do not file federal income tax and were not required to.

Spouse’s Non-Tax Filing Statement
For spouses of students who do not file federal income tax and were not required to.

Verification of Sibling, Spouse or Dependent Post-Secondary Enrollment
This form is requested from those students who need to clarify information concerning siblings, spouses or dependents attending post-secondary institutions. This form must be completed by the registrar at the institution your sibling, spouse or dependent is attending, and returned to the Student Financial Aid Office.

Additional Financial Information and Untaxed Income
This form lists and identifies untaxed income reported on your FAFSA. Verification required from Agency providing support.

Social Security Release
This form is used to request the verification of Social Security Benefits received for students and/or parents.

Welfare Benefit Release
This form is used to request verification of Welfare Benefits received for students and/or parents.

Household Members & Number in College
This form is used to identify student or parent household size for independent and dependent students.

UMDNJ Financial Aid Award Letter
Upon complete review of your financial aid file, the Office of Student Financial Aid will finalize the packaging and awarding process. An award letter will be available for each student to review as follows:

Incoming Students NJMS, NJDS, RWJMS, SOM only
A paper award letter will be mailed to the most current mailing address. Applicants should review this letter. If any changes (increases or decreases) are requested, the applicant MUST make the necessary corrections, sign and return the letter to the office immediately. A revised and/or final award letter will be mailed later in the summer. If the applicant has been given a UMDNJ email address, he/she should regularly check for his/her latest award letter online via the myPortal. Students should be sure to PRINT A COPY of the revised Award Letter for their records.

Remaining Incoming Students and Continuing Student
All award letters for 2010/11 must be reviewed online via the myPortal. If any changes (increase or decreases) are being requested, the student must notify their counselor in writing. Students should be certain to PRINT A COPY of the revised Award Letter for their records.

FUNDS WILL NOT BE DISBURSED UNTIL ALL REQUESTED DOCUMENTATION IS RECEIVED, REVIEWED AND APPROVED.


UMDNJ Appeals Process

1. Your request for additional funds must be submitted in writing indicating the reason for the appeal. If you have incurred additional expenses, attach photocopies of receipts/bills to support your statement.

2. The financial aid staff will review your file and will inform you of their decision.
