Ms. Forbes is a nurse who works with children living with sickle cell disease.  Her dissertation work focuses on the process of moving children with sickle cell from the pediatric to the adult health care system.  This transfer, which occurs at adolescence, is an important and necessary step to delay the morbidity and mortality associated with chronic illnesses.  The development of comprehensive, multidisciplinary transitional care models that promote seamless and effective transfer of medical care from the pediatric to adult healthcare system are urgently needed as such models of care will ensure that access to continued healthcare services is uninterrupted as young adults move from pediatric to adult care.  Through her dissertation project, Ms. Forbes hopes to help develop such a model."
